Concerning Requests for Disclosure

The following shall apply to requests for disclosure, etc. of personal information other than requests for disclosure, etc. of personal information provided to the sales department of Hitachi Solutions, Ltd. (hereafter, the "Company") or requests for disclosure, etc. of personal information on a website of the Company.

1. Request Format

Please download and use the "Request Form for Disclosure, etc. of Personal Information".

If you cannot download the "Request Form for Disclosure, etc. of Personal Information" , the Company can send it to you by facsimile or post.Inquire at the "Inquiry window concerning personal information protection".

2. Attached Documents

Please attach the following 2 documents to the "Request Form for Disclosure, etc. of Personal Information"

  • Identification document (for details, see below)"Concerning Personal Identification Documents"
  • Commission fee payment certificate(Not needed in cases of personal information correction, addition, or deletion)

3. Payment of service charge

For the disclosure or notification of the purpose of use of personal information, a service charge of 800 yen is charged. For details, see "Concerning Method of Payment of Service Charge". There is no service charge concerning correction, addition, deletion, suspension of use, or suspension of provision to third parties of personal information.
Note that the service charge will not be returned when the disclosure or notification of the purpose of use is not performed because the Company does not possess the customer's personal information or for reasons stipulated by law.

4. Request method

The customer is requested to send the materials listed from a) to c) below, in a sealed letter by delivery-certified mail to the address given in 5. below. (The postage is to be borne by the customer.)

6. Response from the Company

The response to the customer will be sent by restricted delivery mail (special type) to the address stated on the customer's application and identification document. The Company requests a grace period of about 2 weeks to conduct an internal check before responding to requests. If about 2 weeks or more is required, the Company will notify the customer of that fact.
